Application Requirements

Please read our Guidelines for Funding as they will save you time when applying.

Please provide:

• Rare disease/condition name and your Medical Specialist's (not General Practitioner or Therapist) email address

• Copy of child's birth certificate 

• Colour copy of Medicare Card

• Proof of citizenship and residency of at least 5 years - child's Australian Passport or Citizenship Certificate 

• Private Health Insurance details

• Government assistance details e.g. National Disability Insurance Scheme (NDIS) Plan and supporting evidence of NDIS approval or decline

• Quote/Invoice for items or services being requested

• Income and expense details

• Tax return/payslip
